Persons 18 years and older complete the Adult Packet.
The Child/Adolescent Packet is for clients under 18. It will be filled out by a parent or parents.
For Your Records is an optional file to read through or print and save for your records. It includes documents such as the full HIPAA Notice of Privacy, Office Information and Fee Schedule, and so on.
